A historic 1966 Aftra Membership Agreement for Captain Beefheart, signed with both his professional name Don Van Vliet and his legal name, Don Vliet. This is the only signature we have ever seen or heard of with Beefheart's legal name. Evidently he added the "Van" very early on, as a tribute to Van Gogh. Musicians joined AFTRA to collect payments due them from recording sessions, and Television and Radio appearances. This dates from Beefheart's first TV appearance, on 9th Street West, April 28, 1966 (with the Magic Band, of course.) Also includes AFTRA receipt. Contract 6" x 8.75;" receipt 3.5" x 6.25."
